Zaloguj sie do phpmyadmin, wybierz twoja baze danych i,
i daj ten kod w sql zakladce i "go" wez.

ALTER TABLE `accounts` ADD `flag` varchar(255) NOT NULL;

Bynajmiej tak mi sie wydaje ze to jest to ze brakuje ci kolumny "flag"
